    To process your deletion request efficiently and securely, please provide:

    Personal Identification:

    Verification Documents:

    • Copy of government-issued photo ID (driver's license, passport, or state ID)

    Processing Timeline

    Standard Processing

    • Acknowledgment: Within 48 hours
    • Verification: 3-5 business days
    • Deletion: 30 days from verification
    • Confirmation: Within 45 days total

    Healthcare Data (PHI)

    • Review: 30 days (HIPAA compliance)
    • Legal Assessment: Additional 30 days if needed
    • Deletion: Upon approval
    • Total: Up to 60 days

    Note: We may extend these timeframes by an additional 60 days when reasonably necessary, and will notify you of any extension within the initial response period.

    What Data Will Be Deleted
