Results, notes and follow-up land in the resident's chart after a clinician reviews them, not before.

What does Gia write back to PointClickCare?

Gia writes the check-in result, the structured assessment when one was administered, the clinician's note and the assigned follow-up to the resident's chart in PointClickCare. Nothing writes back until a clinician has reviewed the result.

IT managers want to know what crosses the boundary into the chart, in what order, and under whose control. This is the sequence.

The cost of getting write-back wrong is a chart nobody trusts. Results that appear without a reviewer's name, assessments that land outside the window, notes that cannot be traced to a person. The sequence below is designed so none of that can happen.

What a check-in produces

Gia calls the resident by phone or video. A conversation runs for a few minutes. Inside it, a 40-second speech check-in measures 2,500 speech biomarkers. If the resident does not answer, Gia retries. A missed check-in is itself a result.

The output is a result on the clinician's ranked worklist, with the reason beside the name. Nothing has touched the chart yet.

The reason is specific. Three missed attempts. A new symptom the resident reported. Speech that has shifted since the last call. A structured assessment the clinician asked for. The clinician can read the reason in one line and decide whether to open the result.

Caregiver check-ins produce results too. The family member who visits most is called on the same schedule, and what they report appears beside the resident's own answers. The clinician reviews both before anything is written.

What writes back, in order

The clinician reviews the result and the Proactive Decision Actions. Options, never orders. The clinician picks one, adds a note and assigns the follow-up.

Then the write-back happens: the result, the clinician's note and the assigned follow-up, dated and attributed, to the resident's chart in PointClickCare. The clinician's review is the gate. There is no path from the call to the chart that skips it.

The note is the clinician's, in the clinician's words. Gia supplies the result and the Proactive Decision Actions. The clinician supplies the judgment. The chart shows both, in that order, with the clinician's name on the decision.

Timing is the same every time. Review, then write-back. There is no batch that runs overnight without a reviewer and no result that reaches the chart because nobody got to it. If a result is not reviewed, it waits on the worklist and the chart stays as it was.

Structured assessments: BIMS, ADL and IADL

When a clinician clicks Initiate Assessment, Gia administers the BIMS, a cognitive screening, or an ADL and IADL review in conversation. Results reach the clinician in under two minutes.

After review, the scored assessment writes back with the result. The MDS coordinator finds it in the chart, in the assessment window, attributed to the date it was administered.

Nothing about the instrument changes. The items are asked as written. The scoring is standard. What changes is who asks, when, and in which language. Gia administers in any of 44 languages, in the language the resident answers in, and retries if the resident does not pick up.

What Gia never writes

Gia places no orders. Gia changes no medication. Gia writes no care plan. A check-in is a screening conversation. Gia is clinical decision support, not a diagnosis.

Whatever appears in the chart was reviewed by a clinician, and the chart says which one.

Gia also does not write speculation. A speech check-in produces a comparison with the resident's previous calls and a ranked position on the worklist. It does not produce a label. The clinician decides what the comparison means for this resident, and that decision is what the chart records.

Epic and other systems

The same sequence runs for Epic and for other EHRs. Call, result, clinician review, write-back. The chart of record stays yours.

Gia operates under HIPAA with a business associate agreement signed with each customer. See integrations for the current list and governance for the data posture.

Gia Health Analytics, the population side, is hosted by Scienza and sold through the AWS Marketplace and Google Cloud Marketplace. Your records never train the model. The two products share one rule: the chart of record is yours, and a clinician stands between every output and every action.

Where to start

Ask your PointClickCare administrator which units should receive check-in results and which clinicians own the worklist. That decision, plus the signed BAA, is the setup.
