Medicare Advantage and D-SNP plans

Fewer members, harder grading, and a book you still have to reach.

Gia calls every member between visits and tells your care managers who to assess first.

What it costs you

Your year is graded on members nobody has talked to.

Stars and quality teams

Mental health outcomes, triple-weighted from 2027.

Measure C05 is back on the Health Outcomes Survey, and cut points keep moving up.

Risk adjustment and clinical documentation

A dementia that is not in an encounter.

Under V28 it is documented in a clinical encounter or not at all. Finding who to assess is the hard part.

D-SNP operations

An assessment and a care plan owed to every dual member.

One HRA by 2027, a care plan within 90 days, and members your staff cannot reach.

The history that points to cognitive decline is already in your data. Nobody is reading it member by member.

How Gia fits your workflow

Gia reaches them. Care management decides. The encounter is yours.

1

Gia reaches the members who never pick up for you.

Phone or video, 44 languages, forty seconds. A missed call is retried.

2

Your care managers get a short list, not a roster.

A ranked worklist with the reason beside each name. One click starts a cognitive screening or BIMS.

3

The encounter happens with your clinician.

Results, options and follow-up write back to Epic or your EHR. The documentation is theirs.

Gia is a Digital Human® with Voice AI. Every result is an option for a clinician to review, not a diagnosis.

What you get

Outreach that happens, and a record that proves it.

Member outreach between visits

Calls to any phone, including a landline. No app, no portal login.

Ranked worklist

Who to reach first, with the reasons. Options, never orders.

Initiate Assessment

Cognitive screening, BIMS, ADL and IADL, administered in conversation.

Caregiver check-ins

The family hears from Gia too, so the change at home reaches your team.

Follow-up record

What the clinician chose and what happened next, kept in one place.

EHR write-back

Epic, PointClickCare or your EHR. Nothing to re-key.

Options for review. A clinician decides.

Behind the call

Scienza's own foundation model for aging reads each member's history first. Tested on 469,000 people it had never seen; five pre-registered outcomes passed. Run it on your own membership.

Reach every member you kept.

Twenty minutes. Your Stars measures, your dual book, your EHR.

Questions

Does Gia perform the health risk assessment?

No. The HRA and the care plan are the plan's obligations. Gia reaches members between visits and gives your care managers a ranked list of who to assess next.

Does Gia raise risk scores?

No. Gia surfaces members whose speech and history suggest a clinician should assess them. Any documentation happens in a clinical encounter, by a clinician.

Does Gia replace care managers?

No. Gia places the calls and listens. Your care managers decide who gets a visit, a referral or a follow-up call.

What do members need to have?

A phone. Video check-ins work on a tablet, phone or TV, but a landline is enough. Gia speaks the language the member answers in.
